Child Care Immunization Report due November 1

This year's Child Care Immunization Reports are due by midnight on Wednesday, November 1, 2023, and will not be accepted after that date.

Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county.

Go to Child Care Immunization Report 2023-2024 to find instructions, a helpful worksheet and a guide to submitting your report through the online portal.
